Health Requirements for Kindergarten and 1st Grade
The District complies with state-mandated health requirements for school entry.
- Immunizations must be complete before kindergarten enrollment paperwork will be accepted.
- Oral Health Assessment - Download Oral Health Form
The Oral Health Assessment form is not needed to start the enrollment process; however, DMUSD asks that these forms be completed before your child begins kindergarten.
Optional Forms
- CHDP Report of Health Examination is an optional resources for families (recommended within 6 months of entry to Kindergarten or 1st grade) - Download CHDP Form
Student Health Conditions
Student Health Conditions
If your child has allergies, diabetes or other health conditions that require school management, physician forms will be required for any procedures or medications needed. If your child takes medication during the school day, physican medication forms must be on file before medications may be stored in the school health office. Hearing and vision screenings will be completed by the school nurse during the year.
