Health Requirements for Kindergarten and 1st Grade
The District complies with state-mandated health requirements for school entry.
- Immunizations must be complete before kindergarten enrollment paperwork will be accepted.
- CHDP Report of Health Examination for entry to Kindergarten and 1st grade must be signed by the doctor performing the physical (done within 6 months of entry to Kindergarten or 1st grade) - Download CHDP Form

The CHDP and Oral Health Assessment form are not needed to start the enrollment process; however, DMUSD asks that these forms be completed before your child begins kindergarten.
Student Health Conditions
Student Health Conditions
If your child has allergies, diabetes or other health conditions that require school management, physician forms will be required for any procedures or medications needed. If your child takes medication during the school day, physican medication forms must be on file before medications may be stored in the school health office. Hearing and vision screenings will be completed by the school nurse during the year.
